MYSTERY SHOPPER APPLICATION

All applicants will be added to a database from which our mystery shoppers will be
selected. Completing this application does not automatically result in a $10 credit
on your bill. Those members selected to be mystery shoppers will be contacted by
the Quality Assurance Analyst.
